About Us

Graceful Transitions is a family-owned local Care Management business supporting older adults and the families who love them through every stage of aging. Our team brings 136 years of collective experience and includes a Certified Senior Advisor, 2-Nurses, a Licensed Social Worker, and 2-Certified Dementia Care Practitioners, partnering with trusted community professionals so families never have to navigate aging alone.

We provide advocacy, education, guidance and support to families who are facing complex challenges in choosing care, identifying problems and mitigating risks. Some may also be struggling with balancing their own lives and managing their loved one's care. Our services are especially helpful for families who aren't nearby and are having difficulty managing their loved one's care from a distance.
