Program Counselor (Case Manager)

We are seeking a Program Counselor (Case Manager) to join the Vista team to coordinate, monitor, and document all case management services for members in our Engage Program.

Responsibilities for a Program Counselor (Case Manager) include:

• Developing written program plans for each member that evaluates progress, determines the individual’s goals, and identifies programmatic service needs
• Implementing systems which support each assigned student’s programmatic and individual goals that address four main areas: social/emotional, vocational, life skills, community involvement (such as recreation and transportation)
• Providing individual and family counseling for members
• Communicating member needs, issues and goals to all staff members and other involved parties
• Maintaining all necessary records and documentation, including monitoring students’ medical, wellness and financial needs

• Bachelor’s Degree plus one (1) year of related experience.  Vista values the work experience of applicants and therefore, may waive the Bachelor level of educational requirement of a position in lieu of a strong work background in a related field
• Valid driver’s license plus a safe driving record
